There are several health forms needed to be submitted throughout a child's elementary years. Each student at every grade level is required by the Iowa Immunization Law to have an immunization form on file with the school. A child without a valid form may be withheld from school until this documentation is received.

Four year old preschool students are required by a statewide grant to have documentation of a physical examination. All other grades a physical examination form is only recommended.

Students entering Kindergarten will also need a Certificate of Dental Screening form as well as a Certificate of Vision Screening form. Children entering the 3rd grade will also require a vision screening.

The dental screening can be performed by a licensed dentist, dental hygienist, physician, physician assistant, RN or ARNP.  A screening anytime after the age of 3 years old will meet the dental screening requirement.

Students entering Kindergarten AND 3rd grade are required to have a vision screening no earlier than 1 year prior and no later than six months after enrollment. An ophthalmologist, pediatrician, physician or professional licensed for this test can perform the screening.
